Where do apply for my Transcript of Records / Diploma / Other Records?

 

You apply at the Registrar’s Office by filling up appropriate Request Forms.

 

 

How much are the charges for such papers?

 

As of SY 2006-2007, the following are the fees for the different records you need:

 

Transcript of Records

P80 (for Nursing)

Facsimile of Diploma P100
Any Certification P50
Honorable Dismissal Certificate P110 (Graduates)
  P100 (Undergrads)
Course Description P50 (1-10 subjects)
  P100 (more subjects)

CAV (Certification/

Authentication/Verification by CHED)

 

P100 (Registered Mail/International)

  P60 (Registered Mail/Local)
  P1,000 (USA destination)

 

                                                                       

When can I get the papers I have applied for?

 

You get your papers according to the following schemes:

  • 10  working days for Transcript of Records

  • 3 working days for Certifications

  • 3 weeks for Certification/Authentication/Verification (CAV)

  • 7 working days for Course Description

  • 14 working days for Facsimile (Xeroxed) Copy of Diploma

 

Can somebody get my records for me?

 

Yes, somebody else can get the records for you, provided you give a Letter of Authorization, together with your ID, to the bearer.

 

 

I am living overseas.  How do I request for my Transcript of Records, Diploma, other pertinent papers?

 

You may request the pertinent records through any of the following:

You may pay such records you requested through any of the following:

  • Authorized person         

  • Check

  • Credit Cards

 

How do I get my Yearbook (Annual)?

 

You may get your Yearbook or Annual through any of following:

Note:  If there are no more copies available, we may scan the book, but at a cost, depending upon the cost of scanning.  It may take a month or so to do this.
